1. Is this light fixture damp-rated? I want to install it in a bathroom about 5 ft away from a shower.

I installed mine in a bathroom within 4ft of the shower and had zero issues in the last 6-8 months. I would suggest avoiding placing it directly over the shower.
2. Can this semi-flush pendant light be used in a kitchen?

It's a good size and looks really pretty with an Edison bulb. I used it in a bathroom, but it would work great in a kitchen too.
3. Would the clear glass fog up in a small bathroom due to steam?

It will depend on the ventilation and the heat from the shower. However, the fog will go away when the steam leaves. I clean mine once every six months, and it still looks fantastic.
4. Is this light fixture bright enough to illuminate a 12x10 bedroom?

Personally, I think it would work great as it's bright, and the clear glass cover helps brighten up the room.
5. I'm planning to use this light fixture in my small office. Does it produce any heat?

It depends on the light bulbs you use. Incandescent lights do produce heat, but LED lights don't produce much. I've had these fixtures in my house for three years now.
